The application is an interactive pdf form. Certain versions of Adobe Reader will not let you save the info you type into the form. You might need to print the completed form and fax it to: 831-459-5717.

Applications are due Dec. 4 and you will receive notification of your status by Dec. 10. Please do not being making travel/accommodation plans until you have received your acceptance notification.
